Registration Information for Returning Students

The registration information below pertains to students who were enrolled at Ballet Austin Academy for the 2009/10 school year and who received a Pre-Registration Packet in the mail.

Please take the following steps to register your child for the 2010/11 Academy school year.
REVIEW THE REGISTRATION FORM FOR CLASS OPTIONS AND TUITION
- CLASS REGISTRATION FORM - All classes offered in your child’s class level and/or age are printed on the Registration Form for returning students. If more than one class offering is available to you, please indicate your first and second choice. Your child will be enrolled into your second choice if the first choice is full. Classes are filled on a first come, first served basis. (If only one choice works for your schedule, please do not choose a second class option)
- Please review your personal contact information on the Registration Form before returning it and make a note of any necessary changes, including spelling of names, birthdates, contact name, addresses, phone numbers and e-mail address. Please note the request for a primary email address to be indicated on the form.
PLEASE NOTE: Some classes fill quickly so registering early is recommended. Class availability cannot be confirmed until registration and payment are processed.

- PAYMENT OPTIONS FORM
- Ballet Austin offers several options to make paying tuition convenient and easy.
- Please refer to Policies and Procedures information before making your tuition payment selection.
- Please note your tuition payment option, fill in pertinent information, and sign the Payment Options Form before returning.
- If you select an automatic payment plan please sign the appropriate Authorization Agreement page on the Payment Options Form for the auto payment plan you select.
- LIABILITY WAIVER/PHOTO LIKENESS RELEASE FORM
- A new form must be completed and on file for the 2010/11 school year.
- In order for your child to begin classes in September, the Liability Waiver must be signed.
RETURN ALL REGISTRATION FORMS TO BALLET AUSTIN
- Please return forms with one of the following:
- Tuition in Full (Pay in Full Discount available through August 26, 2010) See Payment Options Form.
- The non-refundable, first installment. If you choose the interest-free installment plan, we encourage you to choose either the direct payment or monthly automatic credit/debit card payment option so that you will avoid any late fees that might occur.

ADDITIONAL REGISTRATION INFORMATION:
The Schools’ office, located in the Butler Dance Education Center, 501 West 3rd Street, will be open from 8:30 am-5:30 pm, Monday through Friday, throughout the summer. There is also a Schools Receptionist at the Patron Services desk in the front lobby Monday through Thursday until 8:00 pm, and on Saturdays between 9:00am and 3:00pm. We are able to receive registration forms and payments in person, by mail, by fax or by phone during the above hours. To register by fax or phone, make your payment with a Visa, MasterCard, American Express or Discover Card.
Please note: If you register by phone you will need to return the Payment Options Form and the Liability Waiver before the start of the school year.
Confirmation of your registration and payment will be mailed in July. You will be notified by phone only if both your first and second choice of class is full. Outstanding balances from the 2009/10 school year must be reconciled before registration will be accepted for the 2010/11 school year. There will be no exceptions.
If you are interested in enrolling a sibling who has not received a Registration Packet, you may print a New Student Registration Form or call and request class information at 512.476.9051, ext. 138 or 102.

It is recommended that you read through the Policies and Proceduresinformation thoroughly before registering.
